Two chametz sale forms are available. One form is a sale that will take place on the 13th of Nissan, marked 13 on the top left corner. The other is for the regular sale, taking place on the 14th of Nissan, marked 14 on the top left corner.

If you will be traveling for Pesach to a place that is in a different (further east) time zone (for example to Israel), you must fill out a 13 form, because Pesach will begin for you before the regular sale occurs. 

Anyone who will be staying home for Pesach, should fill out the 14 form. 

You can download the form and then print it out and bring it to the office, If you wish and are so equipped, you can download and fill it in on your computer and email it to president@vcjewishcenter.org. In either case, please do so no later than Thursday, April 18 so that it can be gotten to Rabbi Lowenthal in time. Blank forms will also be available in the office, if you prefer.
